Who needs a DS form 4085?
A DS Form 4085 is the US Department of State Application for Additional Visa Passport Pages or Miscellaneous passport services. The form must be completed by an individual when they need to obtain additional visa pages to a valid passport if their current US passport has only 2-4 visa pages left. The opportunity to obtain extra passport pages is necessitated by the fact that some countries may require two or more pages for visa stamps. A request for additional pages may not be processed due to the valid passport’s significant damage (water damage, rips or holes).
If the valid passport is expiring in a year or two it is advisable to apply for a passport renewal rather than adding pages.
Should the DS Form 4085 be accompanied by any other documents?
The Application for Additional Visa Pages must be submitted along with the valid US passport and a payment of the fee required for its processing. The standard fee is $82 for a routine service taking six weeks. If the applicant want to receive their passport back more quickly, the fee for expedited processing is $142.
